Memorial gifts

Celebrate the memory of a loved one with a gift in his or her name. Many families use obituaries and funeral notices to suggest contributions to a specific service or facility in lieu of flowers. For more information call 262-928-4903 or email foundation@phci.org.

How to make a memorial gift

To make a memorial gift online, check the box under “Tribute Gift” in our online donation form. Then select the option “This gift is in memory of” and type in the name of the person you wish to honor.

Tribute gifts

Make a tribute gift in honor of a remarkable person in your life. Or ask others to honor a special occasion, such as an anniversary, birthday or wedding in lieu of gifts. If you’d like help establishing a tribute gift or asking others to donate in your name to honor a special occasion, call 262-928-4903 or email foundation@phci.org.

Thank-you gifts

Recognize an outstanding doctor, nurse, staff member or volunteer at ProHealth with a thank-you gift in their name. Thank-you gifts, or gratitude gifts:

• Help us enhance our programs and services.
