Transportation Guidelines

ProHealth Care's transportation service team is committed to providing safe, quality, comfortable group transportation. Our service is for patients who have no other means of getting to and from their ProHealth Care medical appointments. We also provide transportation for patients who have been discharged from Waukesha Memorial Hospital, Oconomowoc Memorial Hospital and the Rehabilitation Hospital of Wisconsin. We do not provide transportation for emergency care or treatment.

Here is some important information you will need to know:

  • Our hours of operation are Monday through Friday from 7:30 a.m. to 6:00 p.m. We provide transportation service for medical appointments scheduled between the hours of 7:30 a.m. and 3:30 p.m.

  • All requests for transportation should be made 48 hours (two working days) in advance of your appointment.

  • Patients/passengers will be grouped by geographic areas for pick-up and drop-off. Wait times may occur.

  • First-time customers will be contacted by telephone to confirm their appointment date and pick-up time. For subsequent appointments, individuals should be ready one hour prior to their scheduled appointment time.

  • The first time individuals use our service, they will receive a ProHealth Care transportation services information packet.

  • Our fee is $40.00 for one-way service or $80 for a round trip.

  • Monthly invoices will be mailed to your home. Transportation specialists will not collect payment.

  • If your medical appointment changes and you no longer require transportation, service must be cancelled at least 24 hours before your appointment or you will be charged $40.

  • Smoking is prohibited in all ProHealth Care vehicles.

  • No stops for personal business or errands will be made.

  • All passengers will be secured by lap and shoulder belts or be secured in an appropriate car seat.

  • Passengers transported in a wheelchair must be secured by the lap belt if one is present. Wheelchairs must be appropriately-sized and equipped with proper footrests. Brakes should be in good working order.

  • Patient/customer clothing or outerwear must not interfere with the proper securement of lap or shoulder belts.

  • Scooters, lightweight wheelchairs, recliners and some motorized chairs (including Jazzy and Pronto brands) cannot be properly secured and will not be transported.

  • Passengers using a hospital wheelchair must be able to get in and out of the wheelchair or have appropriate assistance available. Hospital wheelchairs cannot be left with passengers.

  • Personal property (such as luggage, gifts, flowers, balloons) should be packaged adequately for safe transport, or be sent home prior to transport in the case of a hospital discharge.

  • Self-administered portable oxygen or oxygen cylinders in a wheeled carrier or cart are acceptable. Patients/ customers requiring IV support or ventilators will not be transported.

  • Children under the age of 8 years old, weighing less than 80 pounds, or less than 57 inches tall will be transported in their own approved child car seat or booster seat to comply with Wisconsin state statutes, and must be accompanied by a parent/guardian.
