Welcome Faculty

Preparing for placement

ProHealth Care is committed to fostering the development of nursing and other health care careers. Academic programs must complete a Student Affiliation Agreement prior to planning student placements. No students will be placed without an agreement.

Rotations (multiple students)

  1. Please have your school liaison submit any requests for placement to Student Affiliations .
     
  2. The placement request acceptance and confirmation deadlines will follow SEWNA guidelines.
     
  3. Upon approval of your request, your school liaison will be directed to add your student information into the ProHealth Care Student Portal at least four weeks prior to your start date.
     
  4. Please contact Student Affiliations to schedule an Epic Computer Training classroom for students accessing the electronic medical record.
     
  5. The following forms are located on the School Faculty Resource page. Document completion in the student portal:
    1. All students must sign and comply to the ProHealth Care Student Confidentiality Agreement
    2. All students must pass the ProHealth Care Student Orientation – Online Session.
       
  6. Schedule a meeting with the Unit/Department Manager to go over student objectives prior to placement.
     
  7. The Instructor Orientation Checklist and Student Orientation Checklist  forms must be completed. Send these completed forms to Student Affiliations c/o Center for Learning and Innovation or fax to 262-928-2092.
    1. Instructors: print off these Orientation Checklists for each of your students. You will use this form as you orient them to their assigned unit.
       
  8. All students must wear a ProHealth Care Student Indentification Badge and a school picture ID. Take the Badge Request Form, along with your student roster to the security office to pick up the badges.

Preceptorship (one-on-one)

  1. Please have your school liaison submit any requests for placement to Student Affiliations.

  6. Please schedule a meeting with the preceptor, student and instructor. The purpose of this meeting is to review the learning objectives of this experience and ensure that questions have been answered.
     
  7. Student prints off and completes Preceptorship Student Orientation Checklist with mentor on first day. Send the completed form to Student Affiliations, c/o Center for Learning and Innovation or fax to 262-928-2092.
     
  8. All students must wear a ProHealth Care Student Indentification Badge and school picture ID. Take the Badge Request Form to security to pick up the badge.
